It is easy to generalize that counseling is common in some countries but limited in Turkey, but it falls short of explaining the actual decisions of institutions. In the same country, there are organizations that produce strong results with external expertise and organizations that only purchase reports. It is often not the cultural label that makes the difference, but how the work is defined and managed.
Outside perspective is particularly valuable in the healthcare industry; because clinical, operational, financial, human resources, quality, legal and technology decisions are interconnected. However, external support will not be permanent if it is seen as a solution that takes over the management responsibility of the institution. It produces consultancy value with in-house ownership and implementation capacity.
The right start: not a solution, but a management question
Projects that start with broad statements like “we want to institutionalize” or “we have a productivity problem” easily fall apart. The first step is to clarify which decision management wants to make better and which outcome it aims to change.
For example, if the problem is "patient waiting", it should be examined which patient group, at which hours, in which process connection and with which result was encountered. If the problem is not concrete enough, the consultancy output remains at the level of general recommendations.
The value that external expertise can provide
Well-structured consulting can make three key contributions to the organization:

Just a list of actions followed by a consultant will not produce corporate ownership. Every job must have an in-house owner, delivery criteria and management decision. The consultant supports the application. It does not make responsibility invisible.
Use data and field observation together
Interviews with managers alone may not show how the day-to-day business runs. Reports alone can also hide definition and data quality issues. Reliable diagnosis. It uses a combination of process observation, employee interviewing, record sampling, and measurement.
It is important that initial findings are discussed openly with management. The purpose of the project is not to validate a ready-made solution, but to understand the real need of the organization.
Move output from report to application
One suggestion. It becomes feasible when the owner, schedule, resource need, success criteria and risk are defined. Testing large transformations with small pilots allows you to see unexpected load and strengthen employee engagement.
At the end of the project, the institution should be able to maintain the method on its own. If the indicator dictionary, meeting rhythm, process ownership and problem-solving skills are not transferred to the institution, external dependency occurs.

- What is the consultant's actual work context in the healthcare industry?
- With what data and field observation will the diagnosis be made?
- How will the sharing of responsibility be established in practice?
- By what criteria and over what period of time will success be monitored?
- What methods and competencies will be transferred to the corporate team?
- How will conflict of interest, data confidentiality and decisional independence be protected?
